How To Register...
By selecting the registration form link (PDF), you may print the
registration form for Best Care Connected. You cannot register for the course
online. Instead, you must print this registration form and mail it to the
address on the form.
Once you have completed the form, you may send it to the address listed on
the form. The course is free of charge, so the completed registration form is
all that we need from you.
When we receive your registration form, we will assign you an I.D. and
password. You should expect to receive a packet containing instructions and your
I.D. and password within 2 weeks of mailing your registration form. You will
receive an instruction packet with your password in the mail after your
registration form is processed .
