Journey to Wellness is a voluntary, comprehensive, year-long program
for employees of Cooperative Extension Service that will help
improve your personal wellness. Activities are designed to allow
maximum flexibility for today’s busy schedules.
Benefits:

- Earn “wellness points” to spend at “wellness store”.
- Receive up to $15 per month reimbursement for participation in
approved wellness activities such as aerobics, water aerobics, gym
memberships, etc. with proper documentation.
- Free screenings for serum cholesterol, glucose, and triglycerides
and blood pressure checks.
- Feel and look better.
- Free body composition analysis and fitness evaluation
- Comprehensive personal wellness profile
- Lots of great information on healthy living and healthy lifestyles
- Tobacco prevention and cessation program
To register Go to the forms site. Print consent form and registration
form. Fill out both and send to Kim Wolfe, Little Rock State Office,
Family and Consumer Sciences. After we receive both completed forms, we'll send you a Personal
Wellness Profile Questionnaire that you'll complete and bring with
you to the kickoff.
Please sign up in advance so that you’ll have plenty of time to
complete your Personal Wellness Profile Questionnaire before the
kick-off events. Impact of the program will be evaluated. All personal information is
confidential and will be kept on a secure computer in the Health
Sciences Department at UALR. Only aggregate data such as 50 people
decreased total cholesterol by average of 5 mg/dl will be reported.
